Step 1: Fill out the SCHEDULE AN APPOINTMENT form above.

• This will let us know that you’re interested in becoming an official member of the practice.

• You’ll be prompted to choose your preferred physician, name, phone number, DOB, & email, as well as some additional information about your history. This helps us better understand where you’re coming from with your health.

• The form will ask for your credit card information, but there are no charges until your first appointment.

• We’re then notified that you’d like to be contacted.

Step 2: We’ll give you a call.

• This is a free consultation “meet and greet” about our practice, rather than a medical appointment.

• Think of it as just a chat so that you can ask any questions you have about our model, what to expect, etc., and decide if we‘re a good fit for you and your family. 

Step 3: We’ll schedule you for a virtual or in-person appointment to establish care.

• That scheduled appointment we set up for the coming weeks would be your first patient visit.
